The next step is to download and submit the forms by clicking on the button below.

In order to reserve various elements of your trip I will need to gather some essential information about each of the travelers, including birth dates, passport details, and any physical limitations. I'll also need credit card information for at least one of the group members. Work on your Trip Plan cannot begin until I have received these forms.

The forms can be filled out on your computer and sent back to me as an email attachment. If you prefer, you can also print them and send them to me via fax, or postal service (the fax number and mailing address are on the forms).

Before I begin work on your Hand Crafted Trip Plan, I require a good faith deposit of $300, equivalent to the minimum of 4 hours work @ $75 per hour. Once I receive your forms, i'll email you an invoice for the deposit.
